The process we follow to issue a medical certificate is exactly the same as if you received one in person.  It is a valid document, and must contain our details, your details, the date of examination, the date of issue of the certificate, and also the date(s) we’ve determined you are unfit for work.  Due to patient confidentiality, we don’t list details of your diagnosis on the certificate (unless you give us express consent).
